Doctor of Medicine | Family Practice Location: Saint Paul, MN Employer: Lancesoft Inc Pay: Competitive weekly pay (inquire for details) Contract Duration: 2 Years Start Date: ASAP About the Position Job Type: Contract / On-Site Provider (Physician or NP) Specialty: Family Medicine, Internal Medicine, or Geriatrics Job Summary We are seeking Physicians and Nurse Practitioners to provide on-site primary care to residents in a skilled nursing facility serving veterans . The provider will deliver high-quality, integrated care , coordinate with nursing staff and the Medical Director, and maintain comprehensive documentation in the electronic medical record. Responsibilities Provide primary care services on-site to residents, including admissions, follow-ups, care plan reviews, and discharge planning Document all patient encounters in the electronic medical record (EMR) Provide after-hours, weekend, and holiday phone/on-call coverage Respond to staff and care team inquiries promptly Coordinate quality-of-care efforts with the Medical Director Participate in policy and program development to maintain high-quality care Maintain a 24/7 coverage plan , including backup providers Ensure compliance with federal, state, and regulatory standards (CMS, Joint Commission, VA regulations) Requirements Qualifications Board-certified or board-eligible in: Family Medicine Internal Medicine Geriatrics Active Minnesota medical license Experience in skilled nursing, long-term care, or similar settings preferred Strong documentation and communication skills Ability to comply with infection control, safety, and occupational standards Must be free from communicable diseases and not disqualified from providing direct care Insurance & Compliance Maintain appropriate professional liability/malpractice insurance Adhere to facility policies and VA regulations Provide care with respect and dignity Care Model Initial visits completed by physician Ongoing care may alternate between physician and nurse practitioner Provide integrated care to promote functional rehabilitation or maintenance Certify medical necessity for admissions, transfers, and non-financial discharges Application Instructions Qualified candidates should submit their CV, board certification, and Minnesota license verification .
